Terms of Registration
Conference attendees will be required to attest that they have read and agree to the following terms during event registration.
Registration
The person completing registration must be an adult or legally emancipated minor. You will be asked to provide payment for your registration via credit/debit card, including registration fees, hotel accommodations, hosted meals, and Kids Camp.
Photography and Video Recording
This conference will be photographed, live streamed, and video recorded, and your image may be captured. Recordings will be made available to the community in various forums, including but not limited to website, YouTube, Facebook, Twitter, Instagram, and TikTok. Cure CMD, A Foundation Building Strength, and Team Titin may also use images for the purpose of marketing or public relations, however, the identities of those captured will not be shared without consent.
In-Person Attendee COVID Protocols
Everyone eligible for the COVID19 vaccine should remain up to date with recommended vaccinations to best protect against COVID19–associated hospitalizations and Emergency/Urgent Care visits.
For in-person attendees, we will follow up-to-date CDC guidelines for large group meetings to promote attendee health and safety for our vulnerable affected community. This will include, but may not be limited to the following protocols at the time of in-person attendee conference check-in:

Proof of full, up-to-date vaccination status, including booster(s), as defined by the CDC on June 1, 2022 for those eligible to receive the vaccination

Alternatively, for those recently recovered from COVID, proof of positive antibody testing dated no earlier June 2, 2022, accompanied by a physician's certification of full recovery from the recent COVID19 infection will be accepted

A photo ID matching the vaccination or positive antibody test record

If vaccinations have still not been approved for children ages 6 months - 4 years, or if there has been insufficient time for children to be fully vaccinated before June 30, additional safety measures may be implemented

These measures are subject to change based on the latest CDC guidance, and may include mask mandates (fully covering nose and mouth), symptom tracking, contact tracing, and onsite testing.
Those who are unable or unwilling to be vaccinated, or show proof of recent positive antibody testing and physician certification are cordially invited to register for virtual attendance. Nearly all sessions will be live streamed and recorded, with opportunities for remote attendees to ask questions and participate in discussions. We thank you for your understanding in our effort to keep everyone in the affected community healthy, and to minimize risk of exposure for our in-person attendees.

Cure CMD, A Foundation Building Strength, and Team Titin bear no liability or responsibility for lost or stolen items, injury, or additional financial obligation as a result of any circumstance related to your conference participation.
